A 21-year-old male presented with blurred vision and irritation in the left eye after a high-velocity metal injury at work. Slit-lamp examination revealed a traumatic cataract with an intact anterior lens capsule and an embedded 1 × 1 mm iron intraocular foreign body (IOFB). Computed tomography confirmed the IOFB. The patient underwent phacoemulsification, foreign body removal, and intraocular lens implantation. Postoperatively, his vision improved to 6/6, with a mild anterior chamber reaction. Intralenticular metallic foreign bodies are rare, often with an entry wound, but this case had no detectable external wound. Early intervention is crucial for preventing complications, highlighting the need for protective eyewear in high-risk professions.
