A 60-year-old female with no personal or family history of breast cancer, menopausal without hormone replacement therapy, presented with ulceration of the left nipple areolar complex for 1 year. On examination, the nipple-areolar complex was completely distorted with an erythematous, desquamative lesion with circular pearly margins. There was no palpable lump and no axillary lymph nodes. The right breast and axilla were normal both on inspection and on palpation.
