Prosthesis infection remains a devastating complication. We sought to assess how commonly used antimicrobials influence growth of bacterial strains isolated from non-infected and infected IPP device biofilms. We also quantified biofilm formation of the isolates using plate-based assays. METHODS: 35 bacterial isolates were obtained from IPP devices explanted for revision or replacement due to mechanical failure, pain, or device infection. Antibiotic resistance was quantified using standard disk diffusion assays for the antibiotics: cefaclor, cefazolin, gentamicin, levofloxacin, trimethoprim-sulfamethoxazole (TMP-SMX), tobramycin, vancomycin, piperacillin/tazobactam (pip/tazo), gentamicin + pip/tazo, gentamicin + cefazolin, and gentamicin + vancomycin. After incubation for 48 hours, zones of inhibition were measured. Resistance breakpoints were obtained via guidelines or published literature. Isolates were also subjected to biofilm formation assays. After an incubation period and removal of planktonic bacteria, crystal violet was added, and absorbance of crystal violet extracted from biofilms was measured at 570 nm using a microplate reader. The mean and standard deviation of the absorbance values were normalized to the OD600 of the bacterial inoculum. RESULTS: Antibiotic resistance results are listed in Table 1. Of the 35 isolates, n=18 were unique species or strains. Most species were resistant to monotherapy, while combination therapy with gentamicin showed varying efficacy. With the exception of Staphylococcus warneri, every species demonstrated the ability to robustly form biofilms on polyvinyl. Corynebacterium qintianiae had the least significant biofilm formation. Staphylococcus capitis formed the greatest biofilm (p>0.0001) amongst all analyzed strains. CONCLUSIONS: Bacterial isolates that form biofilms are commonly resistant to antibiotic monotherapy. Given the strong biofilm-forming capacity of nearly all isolates, infections may occur from an imbalance of symbiotic and dysbiotic bacteria, rather than lack of eradication of biofilm. Further research is required to optimize prophylactic strategies.
