RASP boasts low perioperative morbidity rates with shorter recovery times when compared to more invasive techniques. Same-day discharge (SDD) following procedures like RASP offers patients the potential to maintain the advantages of a minimally invasive approach, while reducing healthcare costs. In this study, we evaluate the outcomes for patients undergoing RASP with SDD. METHODS: Following IRB approval, 34 patients were recruited between July 2021 - July 2023 to undergo RASP with SDD. Patients followed the Enhanced Recovery After Surgery protocol and were provided education regarding SDD before surgery. All patients underwent RASP with an extravesical approach, a 360-degree anastomosis, and received a transversus abdominis plane (TAP) block. Preoperative, perioperative, and postoperative data as well as data from patient phone calls, messages, and hospital visits from the first week after surgery were recorded for analysis. RESULTS: There were no intraoperative complications in our cohort. No patients required continuous bladder irrigation nor drain placement. The mean operative time was 135±35 minutes, median estimated blood loss was 100 (50 – 100) mL, and median resected adenoma weight was 72 (44 - 96) grams. The median time with an indwelling catheter was 7 (7 – 8) days. The median hospital stay was 152 (117-191) minutes, with a median discharge pain score of 3 (0 – 4). Thirty-one (91%) patients followed the SDD pathway. All SDD patients passed trial of void. Two patients were readmitted within 30 days unrelated to SDD: one patient with blood clot retention caused by vigorous exercise in the early recovery period, the other with sepsis secondary to epididymitis/orchitis (patient had history of infections with multiple drug resistant organisms). Four (12%) patients presented to an emergency room, and three (9%) patients communicated with a provider in the week following surgery about the postoperative course. CONCLUSIONS: SDD after RASP is safe and minimizes postoperative hospital time, thus reducing healthcare expenses. Surgical techniques that can help facilitate SDD include a 360-degree anastomosis, extravesical approach, and utilization of TAP blocks.
