Jang , and Saum GhodoussipourSaum Ghodoussipour View All Author Informationhttps://doi.org/10.1097/01.JU.0001008632.59099.b9.14AboutPDF ToolsAdd to favoritesDownload CitationsTrack CitationsPermissionsReprints ShareFacebookLinked InTwitterEmail Abstract INTRODUCTION AND OBJECTIVE: Balancing appropriate antibiotic stewardship while mitigating the risk of infection following prostate biopsies has led to an institutional transition from transrectal (TR) to transperineal (TP) prostate biopsies (bx), forgoing perioperative antibiotic use or bowel preparation. Given this significant practice change, we retrospectively analyzed our patients to compare safety outcomes in TP and TR prostate biopsies. METHODS: A retrospective analysis of 318 patients who underwent TR or TP prostate bx at our institution from 2019-2022 was performed. Patient characteristics and complications rates, including UTI, sepsis, 30-day readmission, and ED visits were collected. RESULTS: We identified 318 patients - 173 had TRbx and 143 had TPbx. Pre- or peri-operative antibiotics were used in 8 patients (5.5%) who had TPbx (due to urosepsis from prior bx or surgeon preference) compared to 174 (100%) with TRbx (p<0.001) (Table 1). Patients with TPbx had no infectious complications, 1 ED visit and no 30-day readmissions compared to 9 (5.17%) infectious complications, 8 (4.6%) ED visits and 7 (4.02%) 30-day readmissions with TRbx (Table 2). Procedure times were shorter with TPbx compared to TRbx (24 vs. 31 min, respectively) and there were significantly fewer total post-operative complications (TR 43.1% vs TP 12.01%). There were similar rates of clinically significant Gleason ≥7 – 62.07% and 61.36% in the TP and TR groups, respectively. CONCLUSIONS: Complication rates, including infectious, were significantly lower with TPbx compared to the TR approach with comparable rates of detection of clinically significant prostate cancer. Despite only a small percentage of patients receiving perioperative antibiotics in the TPbx group, we recorded 0 cases of sepsis or UTI and no 30-day readmissions. With a greater emphasis on antibiotic stewardship, we underline the safety of TPbx which can largely be done without routine perioperative antibiotics or bowel preparation.
