We retrospectively analyzed bile cultures obtained during drainage procedures for acute cholangitis, examining isolated bacteria, their antibiotic susceptibility, and trends over time between January 2019 and December 2023. A total of 416 bacterial strains were identified across 189 cases. Enterococcus spp., Klebsiella spp., and Escherichia coli were the main isolates. While no significant changes were observed in the overall proportions or susceptibilities of isolates, ESBL-producing strains increased significantly. The appropriateness of initial antibiotic therapy did not affect prognosis or length of hospitalization. Healthcare-associated infections were associated with significantly higher resistance rates.
