The establishment of a regional Cath Lab in Zajecar, Serbia, successfully increased access to invasive diagnostics and PCI, particularly for patients with acute coronary syndrome.

Introduction. The Department of Invasive Cardiology in Zajecar was formed in 2014 as part of the network of regional Cath Labs in Serbia. It was founded on the foundations of the Pacemaker Center, which has existed in Zajecar since 2008.Aim. Review of the results of the work of the Department of Invasive Cardiology in Zajecar, in the period from 2014 to 2020.Methods. In the period from 2014 to 2020, 4423 procedures, 2959 (66.90%) coronary angiography and 1464 (33.10%) PCI procedures were performed. As of June 26, 2015, the first approach is a puncture of the right radial artery, and a puncture of the femoral artery is used as an alternative approach.Results. The number of coronary angiographies was 188 in 2014, 470 in 2015, 416 in 2016, 502 in 2017 and 328 in 2018, 561 in 2019, and 494 in 2020. The number of PCI procedures also increased from 36 in 2014 or 19% of all diagnostic procedures, 159 or 33% in 2015, 173 or 41% in 2016, 253 or 50% in 2017, 174 or 53% in 2018, 372 or 66% in 2019, and 297 or 60% of diagnostic procedures in 2020. The number of stents was 47 in 2014 or 1.3 per procedure, 183 or 1.1 per procedure in 2015, 210 or 1.2 per procedure in 2016, 334 or 1.3 per procedure in 2017, 237 or 1.3 per procedure in 2018, 395 or 1 per procedure in 2019, and 409 or 1.3 per procedure in 2020. The ratio of elective procedures to procedures in ACS was as follows 72% electives in 2014, 47% in 2015, 43% in 2016, 30% in 2017, 34% in 2018, 45% in 2019, and 36 % in 2020. In 2014, 28% procedures were in ACS, 53% in 2015, 57% in 2016, 70% in 2017, 66% in 2018, 55% in 2019, and 64% in 2020.Conclusion. More than half of the patients who underwent invasive diagnostics were treated with percutaneous coronary intervention and stent implantation. In the first seven years of work in the Department of Invasive Cardiology in Zajecar, 24/7 on-call system was organized for the treatment of patients with acute coronary syndrome. Thanks to this STEMI network, currently more than half of the patients had been treated for the acute coronary syndrome.
