Abstract A 46-year-old male with celiac disease developed acute back pain following lifting a friend. The pain gradually improved over a few months. Imaging 2 years later showed a compression fracture of the superior endplate of the 11 th thoracic vertebra associated with a Schmorl’s node without bone marrow edema. He had reduced bone mineral density (BMD) in the spine (T-score: −2.4). Trauma as a cause of compression fracture of a dorsolumbar vertebra with a Schmorl’s node is a rare cause of acute back pain, more likely to occur in individuals with low BMD.
