Abstract In mandibular premolars with cervical lesions requiring endodontic treatment, the traditional occlusal access design may weaken tooth structure. A more conservative buccal cervical access cavity can be considered in such cases. Buccal cervical access was initiated in 30 single-rooted mandibular premolars requiring endodontic management. Cleaning and shaping were performed using heat-treated nickel–titanium files (Hyflex CM, Coltene, Whaledent, Switzerland), and the root canals were obturated using gutta-percha and bioceramic sealer (BioRoot RCS, Septodont). Postendodontic restoration was given using fiber-reinforced composite resin (Ever-X Posterior, GC Europe) as the first layer, followed by placement of a hybrid composite (Tetric N-Ceram, Ivoclar Vivadent, Schaan, Liechtenstein) over it. Immediate postoperative pain up to 72 h, and the clinical and radiographic outcomes were evaluated at 3, 6, 12, and 24 months, respectively. At the end of follow-up, all the cases were clinically and radiographically successful. This case series presents successful endodontic management of 30 single-rooted mandibular premolars using buccal cervical access preparation.
