We report a rare case of successful bilateral Toric implantable phakic contact lens (TIPCL) implantation in a 22-year-old male with high myopia and astigmatism, including a pseudophakic right eye after childhood traumatic cataract surgery with a stable posterior chamber intraocular lens. The patient presented with an uncorrected visual acuity of counting fingers at 3 meters in both eyes, improving to 6/9 with high-power correction. Preoperative assessment showed deep anterior chambers, healthy endothelium, and normal fundus in both eyes. TIPCL implantation was performed, resulting in an uncorrected visual acuity of 6/6 bilaterally, with well-centered lenses, optimal vaulting, and no complications. Visual outcomes remained stable during follow-ups. This case demonstrates that TIPCLs can provide a safe and effective refractive solution even in pseudophakic eyes, thereby broadening their role in complex refractive scenarios.
