South African district hospitals play a crucial role in providing healthcare services to underserved communities. However, their operational efficiency varies significantly, necessitating methodological evaluation. We employed a Difference-in-Differences approach to analyse pre- and post-intervention data from selected districts, accounting for potential confounders through regression analysis. The DiD model revealed mixed results with some hospitals showing improvement in resource utilization and patient outcomes after implementing new management strategies. While not all district hospitals experienced efficiency gains, those that did showed a significant increase of up to 20% in resource productivity compared to control districts. District health authorities should prioritise the adoption of evidence-based interventions to further enhance operational efficiencies and service delivery. Treatment effect was estimated with logit (pᵢ) =₀+^ Xᵢ, and uncertainty reported using confidence-interval based inference.
