Abstract Unilateral cardiogenic pulmonary oedema (UCPO) is uncommon, and most cases are due to severe mitral regurgitation. Diastolic regurgitation of the mitral and aortic valve occurs in almost all cases of complete heart block (CHB) (third-degree atrioventricular block) and is usually a benign phenomenon. UCPO associated with CHB is rare. We report a case of CHB with right-sided alveolar opacities as a manifestation of UCPO which resolved within 24 h after permanent pacemaker implantation.
