Abstract

70歳，男性の左鼻腔悪性黒色腫（T4bN0M0（Stage Ⅱ），PD－L1 1％未満，BRAF V600E/K陰性）患者は，重粒子線治療後の全身転移に対し化学療法を開始したが，病状は進行し治療変更を繰り返した。化学療法により一時的な部分寛解を得たものの，左眼周囲において疼痛と倦怠感が増悪した。治療経過中に左眼窩蜂窩織炎および細菌性髄膜炎が急性発症し，迅速な治療介入を行ったが，病状は急速に進行し死亡に至った。髄液検査により細菌性髄膜炎と確定診断され，腫瘍関連の症状に加え，感染症の合併が病状悪化に寄与したと考えられた。皮膚科領域における細菌性髄膜炎の報告は稀であり，本症例は診断の困難性と迅速な対応の重要性を示唆する貴重な経験となった。
