Highlights Thrombo-embolic or haemorrhagic events set in motion a cascade of pathological events eventually leading to mortality in the majority of patients who did not survive open AAA repair. Intraoperative bleeding was a significant contributor to mortality in the ACT-group. Bleeding-and thrombo-embolic complications must be addressed to reduce mortality after elective open AAA repair. Adjudication committees aid in scrutinising complex data of surgical trials.
