Objectives: To evaluate the nationwide volume of surgical procedures for chronic venous disease performed in Brazil between 2015 and 2023 and to compare public and private healthcare systems regarding procedure rates, financial investment, and in-hospital mortality. Materials and Methods: This retrospective nationwide analysis used Brazilian public and private healthcare databases to assess varicose vein surgeries rates, demographic disparities and in-hospital mortality. Data included 1, 211, 689 procedures, analyzed via generalized linear models (Gamma/Poisson distributions). Results: A total of 1, 211, 689 varicose vein surgeries were performed in both public and private healthcare systems in Brazil between 2015 and 2023, with an average surgery rate per 10, 000 inhabitants per year of 3, 73 in public and 14. 52 in private. The total financial investment amounted to US 488, 065, 062. 39with an overall average cost per procedure of US 384. 30. The mean cost per procedure in public was US 194. 70, whereas in private, it was US 573. 90-2. 87times higher than in public. The overall mortality rate was 0. 012%. Conclusions: A total of 1, 211, 689 surgeries were performed between 2015 and 2023 for chronic venous disease treatment, with an average of 6, 43 procedures per 10, 000 inhabitants per year. The number of surgeries in the private system was 3. 36 times higher than in public, considering the number of procedures per 10, 000 inhabitants.
