Opuntia ficus-indica mucilage has emerged as a valuable natural polysaccharide excipient for the development of pharmaceutical matrix tablets, owing to its distinctive rheological characteristics and excellent biocompatibility when compared with synthetic polymers. This review provides an in-depth overview of the existing extraction processes, analytical characterization methods, and functional roles of this mucilage in controlled drug delivery systems. Its remarkable ability to form gels, exhibit viscosity that varies with concentration, and dis play shear-thinning behavior makes it highly effective for use in sustained-release formulations. The polysaccharide components of Opuntia ficus-indica mucilage are mainly composed of arabinose, galactose, xylose, and galacturonic acid units, which together create complex macromolecular structures with molecular weights between 20,000 and 43,000 Da. Detailed analyses have shown that this mucilage exhibits pH-responsive swelling, strong mucoadhesive characteristics, and excellent ability to regulate drug release. Moreover, recent advancements in its modification methods—such as chemical grafting and the development of composite systems—have broadened its applications to include superporous hydrogels, nanoparticle-based delivery systems, and bioadhesive film formulations. The environmentally sustainable nature and economic feasibility of Opuntia ficus-indica mucilage make it a compelling substitute for synthetic excipients, especially in emerging pharmaceutical sectors. Recent studies have reported its successful integration into matrix tablet formulations for a variety of therapeutic drugs, resulting in improved patient adherence and minimized side effects. Ongoing research emphasizes the need for standardized production protocols, stringent quality assurance practices, and innovative formulation approaches to enable its use in personalized medicine. Overall, this review consolidates current insights and highlights future opportunities for the advancement of mucilage- based drug delivery technologies.
