Milagai Thailam is a classical Siddha medicated oil described in Pathartha Guna Vilakam (Moola Vargam) by C. Kannusamy Pillai. Primarily indicated for the management of all types of headaches (Thalaivali), This review examines the synergistic relationship between its primary ingredients: Capsicum annuum, Piper nigrum, Cow milk, and Sesame oil. By analyzing the phytochemical constituents-specifically capsaicin and piperine and their pharmacological actions, this article elucidates how the oil bath procedure facilitates pain relief by improving cranial circulation through the traditional practice of a weekly oil bath.
