This paper presents Swasthya Chetna, an AI-driven digital healthcare ecosystem designed to improve preventive healthcare management. The platform integrates health score analytics, provider networking and grading, financial support through a credit mining wallet, insurance management, educational learning systems, and healthcare marketplaces. The system architecture uses React.js, Node.js, Python Flask, MongoDB, and PostgreSQL with machine learning models for predictive healthcare analytics and provider evaluation. Experimental evaluation demonstrates improvements in healthcare accessibility, patient engagement, and preventive care monitoring through an integrated digital health platform.
