This case study describes the experience of a 66-year-old gentleman who received an autism diagnosis in later life, which was identified through engagement with his community mental health team following being referred into the service for his mood and later to psychology for formulation. This article explores his journey through assessment and receiving a diagnosis, the personal impact of receiving a diagnosis at this stage of life, and the clinical implications for his ongoing care. It also highlights systemic and attitudinal barriers that may delay or prevent access to autism assessments for older adults. Through this case example, the article aims to highlight William’s experience whilst also increasing awareness of late-diagnosed autism in older adulthood and emphasise the importance of accessible and age-inclusive diagnostic pathways within older adult mental health services.
