Aim: This study aims to determine the success and survival of apical resection and identify the factors influencing these. Material and Methods: Patient records of individuals who underwent apical resection surgery between January 2018 and December 2022 at our clinic were retrospectively reviewed. Parameters investigated in this study included age, gender, tooth localization, retreatment, retrograde filling, resurgery, restoration type, presence of post-core, association with the pathological lesion, systemic disease, and follow-up duration. Apical resection was assessed for success and survival. Data were analyzed using SPSS software (version 23.0, IBM Corp., Armonk, NY, USA). Pearson Chi-Square test, Bonferroni correction, and Binary logistic regression analysis were used for analysis. Results: Within the specified years, apical resection surgery was performed on 654 teeth of 440 patients. The survival rate was determined as 97.4% whereas the success rate was determined as 95.4%. Parameters significantly affecting survival were tooth type and resurgery, while gender and follow-up duration were identified as influencing factors for success. Conclusions: Within the limitations apical resection surgery represents an effective surgical treatment option with high success rates. In apical resection procedures, survival is highest in maxillary incisor teeth. It has been observed that the resurgery diminishes survival. Success is lower in females compared to males. Long-term follow-up appointments should be scheduled for patients.
