Aim: To evaluate the early glycemic, weight, and metabolic effects of the fixed-ratio combination of insulin glargine/lixisenatide (iGlarLixi) in patients with uncontrolled type 2 diabetes mellitus (T2DM) despite treatment with oral antidiabetic agents (OADs) and/or single or multiple insulin regimens, and to generate real-world evidence regarding its clinical effectiveness.Methods: This retrospective single-center study included adults with T2DM who initiated iGlarLixi at Burdur State Hospital between June 2024 and January 2025 and used it for at least three months. Patients aged ≥ 18 years with body mass index (BMI) 35 kg/m² and hemoglobin A1c (HbA1c) ≥ 7% were included. iGlarLixi was administered once daily (100/33 or 100/50) with dose titration to fasting glucose targets. Baseline and 3-month measurements of weight, BMI, HbA1c, fasting glucose, lipid profile, and routine laboratory parameters were compared.Results: Among the 65 individuals enrolled, 62 were able to complete the 3-month course of iGlarLixi therapy. During follow-up, we noted reductions in body weight, BMI, HbA1c, fasting plasma glucose, and triglycerides, whereas high-density lipoprotein cholesterol showed a significant rise.
