B-cell acute lymphoplastic leukaemic (B-ALL) rarely presents acutely with leukaemia cutis. We present a case of a previously healthy 47-year-old woman who presented with reticulated purpura on her limbs. She eventually had a diagnosis of Ph-negative B-cell acute lymphoblastic leukaemia (B-ALL). This case highlights an uncommon dermatologic manifestation of B-ALL, underscoring the importance of skin findings in early leukaemia diagnosis.
