ABSTRACT Objective: To describe how case managers can adapt their assessment practices to intentionally engage people living with aphasia in both the assessment and care planning process. Primary Practice Setting(s): Home and community care Conclusion: Case managers can support meaningful engagement for people living with aphasia by using evidence-based strategies and aphasia-friendly communication. A skilled, encouraging assessor amplifies the voice of the person with aphasia through a person-centered assessment and care planning process incorporating client input, caregiver insights, observational cues, and clinical documentation.
