Intraocular foreign bodies and traumatic cataract are important causes of post traumatic vision loss. A 17 year old male was going to school, when some unknown object hit his left eye. The patient remained asymptomatic for fifteen days after which he started noticing diminution of vision. On Slit lamp examination, there was no corneoscleral entry wound. A defect was seen in the iris at 9 o clock position suggestive of entry of some foreign body. Also, a traumatic cataract developed in the lens. Patient was taken for phacoemulsification cataract surgery, a foreign body was found behind the lens in the capsular bag. The foreign body was removed and an IOL was implanted in the bag. This is rare case of foreign body in lens capsule. Also, asymptomatic cases can be misleading for the management hence every patient with history of trauma should have a thorough, in-depth workup.
