Cranial Ultrasound and Magnetic Resonance Imaging in Hypoxic‐ischaemic Encephalopathy: A Comparison With Outcome

SUMMARY Forty term infants with hypoxic‐ischaemic encephalopathy were assessed during the neonatal period with cranial ultrasound and MRI, and the findings were compared with outcome at one year of age. 38 had abnormalities on ultrasound and all had changes on MRI. The incidence of changes in the basal banglia/thalami and periventricular white matter was much greater with MRI than with ultrasound. Changes in the basal ganglia and thalami on MRI were associated with a poor outcome if they had also been detected with ultrasound. However, MRI identified four small infarcts which were not detected by ultrasound. There was no consistent association between periventricular white matter change on MRI and outcome. Regular ultrasound scanning identified all infants with a poor outcome. A normal ultrasound or isolated findings of intraventricular haemorrhage, subarachnoid haemorrhage or transient flares were associated with a normal outcome in 13 of 14 infants. RÉSUMÉ Echographie transcränienne et IRM dans l' encéphalopathie hypoxique‐ischémique: une comparaison avec le devenir Une échographie transcrånienne (US) et une IRM ont été pratiquees durant la période neonatale chez 40 nourrissons nes a terme, et les donnees ont été comparees avec le devenir a I'age d'un an. Tous les nourrissons presentaient des modifications a 1'IRM et 38, des anomalies US. L' incidence de modifications dans les noyaux de la base et thalamus d'une part, la substance blanche periventriculaire d'autre part est apparue bien plus élevée avec 1'IRM qu'aux US; Les modifications des ganglions de la base et du thalamus a 1'IRM, etaient de mauvais pronostic si elles etaient egalement detectes aux US. Cependant, 1'IRM identifia quatre petits infarcissements non detectes par les US. II n'y avail pas d'association significative entre les modifications de la substance blanche periventriculaire et Ie devenir. Des explorations US régulières identifierent tous les nourrissons avec mauvaise evolution. Une US normale ou des donnees isolées d' hémorragie intraventriculaire, d'hemorragie sous arachnoïdienne ou des modifications transitoires etaient associees a un devenir normal chez 13 parmi 14 nourrissons. ZUSAMMENFASSUNG Craniate Ultraschall‐ und Magnetresonanz‐unlersuchungen bet hypoxisch‐ischdmischer Enzephalopathie: ein Vergleich mil Langzeit‐ergebnis 40 reife Neugeborne mit hypoxisch‐ischämischer Enzephalopathie wurden in der Neonatalperiode mit Ultraschall (US) und MRI auf cérébral Veränderungen untersucht und die Ergebnisse wurden mit dem Outcome im Alter von einem Jahr verglichen. Die lnzidenz von Veränderungen in der Basalganglien/Thalamus‐Region und in periventrikularen weißen Substanz war im MRI wesentlich großer als im US. MRI Veränderungen in den Basalganglien und im Thalamus, die ebenfalls im US nachgewiesen wurden, waren mit einem schlechten Outcome verbunden. Im MRI wurden jedoch vier kleine Infarkte nachgewiesen, die im Ultraschall nicht festgestellt werden konnten. Es gab keine direkte Beziehung zwischen MRI Veränderungen in der periventrikularen weiBen Substanz und dem Outcome. Durch regelmaßige US Untersuchungen mit einem 5 und 7‐5 MHz Schallkopf konnten alle Kinder mit einem schlechten Outcome herausgefunden werden. Eine normale US Untersuchung, oder isolierte Befunde einer intraventrikularen oder subarachnoidalen Blutung, oder transitorischc Veränderungen waren bei 13 von 14 Kindern mit einem normale Outcome verbunden. Comparación del curso posterior Cuarenta lactantes a término con hipoxia‐isquemia fueron evaluados durante el periodo neonatal con ecografia craneal (EC) i IRM y los hallazgos fueron comparados con el curso al año de edad. 28 tenian anomalias en la EC y todos tenian cambios en la IRM. La incidencia de cambios en los ganglios basales/talamo y en la substancia blanca periventricular era mucho mayor en la IRM que en la EC. Los cambios en los ganglios de la base y el talamo en la IRM iban asociados a un curso pobre si tambien se habian detectado en la EC. Sin embargo, la IRM identified cuatro pequeflos infartos que fueron detectados por la EC. No hubo una asociacion consistente entre los cambios en la substancia blanca periventricular en la IRM y el curso. El examen regular con EC identificaron todos los lactantes con un curso pobre. Una EC normal o hallazgos aislados de hemorragia intraventricular, o subaracnoidea o de destellos transitorios iban asociados a un curso normal en 13 t de los 14 lactantes.
