This in addition to the complex nature of sexual dysfunction results in a scarcity of interventions. We hypothesize several barriers to TTh prescription for HSDD in the US and aim to investigate TTh prescribing patterns to gain a better understanding of the management of HSDD in women, which remains an understudied diagnosis. METHODS: We conducted a cohort analysis of 18–70-year-olds diagnosed with HSDD (ICD-10 F52.0) via TriNetX Diamond. We excluded those with a diagnosis of intersex surgery, personal history of sex reassignment, gender identity disorders, or a TTh prior to the first HSDD diagnosis. We analyzed trends of TTh prescriptions, routes of TTh administration, and co-administration with estrogen. Sub-group analyses were conducted for various age groups, and Pearson's chi-square test was used for trend analysis. RESULTS: We identified 33418 females diagnosed with HSDD at a mean age of 44.2±10.8 years. 850 females received TTh: 378 received 1 prescription, 195 received 2-3, 161 received 4-9, and 116 received more than 10. 461 females received prescriptions for injectable testosterone, 257 for topical, and 119 for pellets. 162 females were co-prescribed estrogen. A positive quadratic trend in prevalence of TTh for HSDD is observed across all age groups until September 1, 2015, after which there is an observable decrease in prevalence compared to the expected forecast (Figure 1). A positive linear trend in prevalence emerged from this point onwards. The slope of this linear trend is 0.0005 for the overall population (p<0.0001), 0.0004 for 18–40 year-olds (p=0.0002), 0.0004 for 56-70 year-olds (p=0.011), and 0.0009 for 41-55 year-olds (p<0.0001). CONCLUSIONS: Initial findings demonstrate 2.5% of those with HSDD were prescribed TTh, with highly variable duration, route, and co-administration with estrogen patterns. Moreover, long-term adherence to TTh remained low despite an increase in prevalence. With rising TTh prescriptions for HSDD, there's a crucial call for investigating treatment mechanisms, efficacy, and long-term outcomes. This demand, though not new, is now more urgent than ever and underscores the pressing need for further research in women's sexual health.
