District hospitals in Kenya play a crucial role in healthcare delivery, but their efficiency varies significantly. A comprehensive search strategy was employed across multiple databases including PubMed, Embase, and Cochrane Library. Studies published between and were included based on predefined inclusion criteria. The review identified a notable trend towards improving resource allocation through targeted interventions, with some studies reporting efficiency gains of up to 25% in certain areas. While the findings suggest potential for enhancing district hospital operations, there is variability across different regions and healthcare specialties. District hospitals should prioritise standardised training programmes and quality improvement initiatives based on evidence from randomized field trials. Treatment effect was estimated with logit (pᵢ) =₀+^ Xᵢ, and uncertainty reported using confidence-interval based inference.
