Introduction and Objective: Automated insulin delivery (AID) systems using carb counting, sensitivity factor presets for bolus dosing require more user input compared to weight input-initiated AID using AI based adaptive algorithm to autonomously bolus with no manual correction bolus. We compared the two systems in terms of time in range (TIR) and glucose management indicator (GMI) Methods: Weekly trend in GMI and TIR was assessed for 4 weeks after AID initiation in an endocrine clinic. AID and CGM device data downloads were analysed. 8 patients on conventional AID and 8 patients on AID with autonomous bolusing option were studied. Independent and paired t test analysis used Results: There was no significant difference between the baseline GMI/TIR between the two groups. No significant intergroup differences in GMI / TIR seen at 4 weeks. Intragroup analysis showed no significant GMI / TIR change at 4 weeks compared to baseline within user-programmable AID. The AID with autonomous bolusing demonstrated significant GMI / TIR reduction (p value = 0.003) at 4 weeks Conclusion: In this real world comparison, the AID system with autonomous bolusing using AI based adaptive algorithm with limited user input and lower user burden showed greater intragroup improvement in GMI and TIR within the first week.
