1572-P: Clinical, Social, and Psychosocial Outcomes among Adults with Type 1 Diabetes in a Safety-Net Setting

Introduction and Objective: Despite advances in type 1 diabetes (T1D) care, socioeconomically disadvantaged populations often experience inequitable access to diabetes management resources and are at risk for poor outcomes. Recent efforts have focused on improving representation of diverse populations living with T1D in outcomes research to better understand gaps in care, though data from safety-net systems are limited. Methods: A T1D registry was established within an urban academic health system, anchored in a safety-net diabetes clinic. Demographic and clinical data were collected from participant surveys and the EHR. Surveys also assessed diabetes distress, food insecurity, quality of life, fear of hypoglycemia, and health and digital literacy. Results: Participants (n=68) had a mean age of 38.9 ± 13.6 years, HbA1c 9.2 ± 2.4%, BMI 25.5 ± 5.5 kg/m2, and median T1D duration of 17 yrs IQR 11-27. Most identified as Black/African American (87%). Government insurance (28%) and no insurance (26%) were common. Clinical and psychosocial characteristics are summarized in the figure. Conclusion: Adults living with T1D receiving care primarily in a safety-net setting experience high clinical complexity and psychosocial burden. Despite improved access to diabetes technologies, disparities persist. Population-specific data are needed to identify drivers of outcomes and inform context-responsive interventions. Disclosure L.
