Motivation: The underlying mechanisms of cognitive impairment related to glymphatic clearance based on the structural disruption of GM and glymphatic metrics are required. Goal(s): To understand the relationship between structural changes in the GM, glymphatic flow from DTI-ALPS and gBOLD-CSF inflow metrics to clarify the correlations among CSF circulation, gray matter structural changes, and glymphatic clearance. Approach: Comparing glymphatic-related DTI metrics and global BOLD-CSF inflow between the HC and MCI/AD groups, we also examined the correlation between these metrics. Results: Between the HC and MCI/AD patients, a significant difference in DTI metrics and gBOLD-CSF, and significant correlations between DTI metrics and gBOLD-CSF were observed. Impact: These findings highlight the link between glymphatic dysfunction, CSF circulation, and neurodegeneration, offering clinicians and scientists potential biomarkers for early detection. This study paves the way for further research into therapeutic strategies targeting brain clearance systems to mitigate cognitive decline.
