METHODS: Four prospectively maintained dataset from tertiary referral centers were merged and queried for "RAPN" and "Hugo", performed since October 2022. A total of 52 patients were enrolled (Retroperitoneal, n=24; Transperitoneal, n=28). Trifecta was defined as the coexistence of: negative surgical margin status, no Clavien-Dindo grade ≥3 complications, eGFR decline ≤30%. In case of on-clamp procedure, a warm ischemia time (WIT) <25 min was considered mandatory for trifecta achievement. Continuous variables were presented as median and IQR and compared by means of Student's t test, while categorical variables were presented as frequencies (%) and compared by means of χ2 test. RESULTS: All RAPN were successfully performed. Median BMI was significantly higher in the transperitoneal group (68 vs 62.5 years, p=0.003) (Table 1). Similarly, Charlson Comorbidity Index resulted to be statistically significant higher in the transperitoneal cohort (4 vs 3, p=0.01), as well as clinical tumor size (34 vs 19 mm, p<0.001). With respect of preoperative renal function, retroperitoneal cohort showed a higher serum creatinine (1.07 vs 0.94 mg/dl, p<0.001), even if eGFR was comparable between the two groups (p=0.07). Regarding postoperative data, retroperitoneal group displayed a higher docking time (5 vs 7 min, p=0.02) and console time (195 vs 93.5 min, p<0.001) (Table 2). Difference in hilum approach was statistically significant since on-clamp procedures were mostly performed in the retroperitoneal group (20 vs 3, p<0.001). At the final histopathological report, the retroperitoneal cohort showed a higher rate of clear cell RCC (62.5% vs 53.6%, p<0.001). Trifecta achievement rate for retroperitoneal and transperitoneal cohorts were 83.3% and 85.71% (p=0.81), respectively. CONCLUSIONS: Regardless of the surgical route chosen, RAPN using the novel Hugo™ RAS System displayed satisfactory perioperative functional and oncological outcomes. Nevertheless, when considering a retroperitoneal approach higher docking and console time should be expected.
