We aimed to describe perioperative outcomes of the largest multicentric series of RAPNs performed with the new HugoTM RAS System. METHODS: Four prospectively maintained dataset from tertiary referral centers were merged and queried for "RAPN" and "Hugo", performed since October 2022. A total of 52 patients were enrolled. Baseline and perioperative data were collected. Trifecta was defined as the coexistence of: negative surgical margin status, no Clavien-Dindo grade ≥3 complications, eGFR decline ≤30%. In case of on-clamp procedure, a warm ischemia time (WIT) <25 min was considered mandatory for trifecta achievement. A two-sided p-value <0.05 was considered statistically significant. RESULTS: All patients successfully underwent RAPN without the need of conversion to open surgery or additional port placement. Median age and BMI were 64 years (56-72) and 26 kg (24.2-27.7), respectively (Table 1). Median clinical tumor size was 27 mm (17-38.5). Most patients (40.4%) presented with left renal mass and 8 (1.5%) patients had cystic features at preoperative imaging. Regarding postoperative data (Table 2), median docking time and console time were 5 min (4-6) and 145 min (75-200), respectively. Median estimated blood loss was 100 ml (250-300). With respect to postoperative complications, four patients displayed Clavien-Dindo II (single-unit blood transfusion or fever) while four patients experienced Clavien-Dindo IIIa (one lymphocele, two pneumothorax and one pseudoaneurysm). At discharge, median serum creatinine and eGFR were 0.95 mg/dl (0.82-1.16) and 72.1 ml/min/1.73 m2 (61.3-89.7) were comparable to preoperative values. At final histopathological report, most patients (57.7%) presented with Clear cell RCC while only 2 (3.9%) had positive surgical margins. Overall, Trifecta was achieved in 47 (90.4%) of patients. CONCLUSIONS: This study represents the initial multicentric series demonstrating the application of Hugo™ RAS system in the context of RAPN. The promising outcomes and cost-effectiveness may lead urologists to use Hugo Ras system in the nephron-sparing surgical landscape.
