Alongside advancements in the treatment landscape for HCM, diagnosis rates around the U.S. are increasing. Currently, most patients with HCM receive treatment at centers of excellence, however, these institutions are not easily accessible to all patients. A tool like EARTH-HCM can help both patients and healthcare providers understand current geographical disparities in the treatment of HCM as can hopefully lead to improved recognition, care and outcomes.

A sobering finding is that despite the availability of highly effect medical therapy, it is underutilized. Only about 18% of patients with HFrEF are on all four pillars of guideline-directed medical therapy. Fewer than 1% reach target doses. Full implementation could save roughly 95,000 lives a year in North America. So the gap is not mainly a gap in knowledge or in drugs. It is a gap in implementation, access and care delivery.

These 124 evuzamitide data demonstrate the potential of a single test to help health care providers identify or exclude cardiac amyloidosis. Clinically, a sensitive, specific and quantitative PET tracer could help distinguish cardiac amyloidosis from other causes of heart failure, differentiate amyloid subtypes when used alongside appropriate laboratory testing, and track changes in cardiac amyloid burden over time.

By directly comparing aficamten and metoprolol, the MAPLE-HCM trial expands our understanding of how aficamten may be optimally integrated into the management of patients with obstructive HCM. Currently, myosin inhibitor therapy is recommended as a second-line treatment for patients with persistent symptoms on beta-blockers. But here we show that aficamten—as monotherapy and as first-line therapy—demonstrated greater improvements in exercise capacity and symptoms than beta-blockers.

Acoramidis demonstrated early and sustained clinical efficacy on the totality of cumulative cardiovascular outcomes, where accrued events start to numerically diverge within the first month of treatment. As a practicing cardiologist, these findings are incredibly meaningful because it draws attention to the time-sensitive nature of transthyretin amyloidosis diagnosis and treatment initiation, where a safe and effective treatment such as acoramidis can potentially have an early effect on reducing patients' risk of cardiovascular hospitalizations and events.
