Evidence gap
The answer is genuinely unknown — a pending outcome trial will settle it.

If the new survival signal holds, thousands of elderly patients with a leaky tricuspid valve get a procedure that keeps them out of the hospital — if it doesn't, they get an expensive fix that only makes them feel better for a while.

The debate
11 clinicians quoted on the record
A randomized trial now shows fewer deaths and heart failure admissions out to three years, the procedure is safe in trained hands, and elderly patients with severe leak deteriorate while we wait for a perfect trial.
“I think most people would agree, as clinicians, heart failure hospitalization is one of the worst things that can happen to a patient.”
“This resulted in a relative reduction of 60% of the combined primary endpoint of mortality and heart failure hospitalization, with a highly statistically significant P value of 0.0001.”

The survival signal comes from a small, open-label trial at a handful of expert German centers where patients knew they were treated and drug therapy may not have been pushed hard — every prior program in this space delivered better symptoms and nothing more.
“For patients with severe tricuspid regurgitation, transcatheter tricuspid-valve replacement was superior to medical therapy alone for the primary composite outcome, driven primarily by improvements in symptoms and quality of life.”
“To me, that’s going to be the Achilles’ heel of valve replacement [and] figuring that out.”

“What both of these studies provide is an extension of what we saw from the pivotal trials. We continue to see safety, [and] we continue to see efficacy in the real world.”
Where the field stands
Common ground
Severe tricuspid leakage makes patients miserable, and medicines alone do little to fix it.
The crux
Is a symptom benefit from unblinded trials enough to justify a procedure with real pacemaker and bleeding costs, before anyone has shown it saves lives?
Still unknown
A sham-controlled trial, or one powered for death and hospitalization, would settle whether the benefit is real and lasting.

evidence not definitive for health status
“TRILUMINATE and TRISCEND II did not provide definitive evidence of the beneficial impact of the procedures on health status.”

skeptical / not so fast
“I have the TRILUMINATE trial, which was one of the most biased trials in all of cardiology, because one group got the procedure, the other group got tablets, and the primary endpoint was driven only by quality of life changes.”

TTVR helps mortality (crossover analysis)
“I'm starting to feel a lot more comfortable saying that [TTVR] does help with mortality.”

Where the evidence stands
TRISCEND II win ratio favored TTVR plus meds over meds alone, driven by KCCQ and NYHA, not HF hospitalization. Severe bleeding (~15% vs 5%) and new pacemakers (~17% vs 2%) were substantially higher. TEER trials (TRILUMINATE, TRIC-I-HF) similarly improve symptoms with delayed hard-outcome signals. FDA approved Evoque; critics say we are treating a QoL endpoint with a morbid device.
