B-53 | Baseline Anemia And Outcomes in Mitral Transcutaneous Edge-To-Edge Repair With MitraClip Therapy: A Meta-analysis

Anemia is known to be associated with adverse outcomes in several cardiac procedures. Recent data suggest adverse outcomes in patients with baseline anemia undergoing mitral transcutaneous edge-to-edge valve repair (TEER) with MitraClip. However, data was not conclusive. We conduct a meta-analysis to explore the association between baseline anemia and outcomes in TEER. We conducted a literature search from inception through January 2024 in EMBASE and MEDLINE databases. We included studies that report TEER outcomes in patients with and without baseline anemia. Baseline anemia is defined by hemoglobin level of < 13 g/dL in men and < 12 g/dL in women. Data from each study were combined using the random-effects, generic inverse variance method of DerSimonian and Laird to calculate odds ratio (OR), and 95% confidence interval (CI). Four studies, involving 1,351 patients, were included in the meta-analysis. Baseline anemia is associated with increased risk of mortality (OR 1.98, 95% confidence interval (CI) 1.35-2.89, I2 0.0%, p-value 0.00). There is no significant difference in procedural access or vascular complications in patients with and without baseline anemia (OR 0.64, 95% CI 0.08-5.28, I2 0.0%, p-value 0.68; and OR 1.92, 95% CI 0.77-4.75, I2 0.0%, p-value 0.16, respectively). Baseline anemia is associated with up to 2-fold mortality after TEER. There is no significant difference in procedural success or vascular complications in patients with and without baseline anemia.
