Emphysematous pyelonephritis is a rare and severe necrotizing upper urinary tract infection associated with gas formation. We presented the case of a female patient, diabetic with poor glucose control with a 4-day history of slight behavioral changes and persistent abdominal pain. Despite having signs of multiple organ dysfunction, the patient was stable with non-advanced support. Computed tomography (CT) scan demonstrated complete damage to the left kidney by gas. A total nephrectomy was performed, and the patient had a complete recovery after being discharged.
