Abstract Background Tadalafil represent the gold standard for the treatment of erectile dysfunction (ED). Moreover, PDE5 inhibitor is proved to be effective also for the treatment of Lower urinary tract symptoms (LUTS) in several preclinical and clinical trials, since they were found to be able to increase oxygenation and blood supply, reduce intra prostatic inflammation and reduce the smooth muscle tone of the Lower urinary Tract. However, it has been suggested that a single drug is not the ideal therapy for relieving LUTS, especially as measured by the Storage International Prostate Symptom score (SIPPS). Therefore, the opportunity to treat LUTS by using tamsulosin alone or in combination with tadalafil may allow new therapeutic Strategies Aim of Work To evaluate the efficacy and safety of tamsulosin (0.4 mg) alone or in combination.with tadalafil (5 mg) for the management of LUTS Patients and Methods This study was randomized controlled trial clinical study that underwent in urology department, Faculty of Medicine, Ain Shams university in the period of 4 months over 120 divided into two groups: Group A: 60 patients received Tamsulosin 0.4 mg once daily with placebo. Group B: 60 patients received Tamsulosin (0.4 mg) in combination with Tadalafil (5 mg) once daily Results and Conclusion Tamsulosin in combination with Tadalafil allows achieving an additional significant improvement of International Prostate Symptom Score (IPSS) and relieving benign prostatic hyperplasia symptoms as compared to Tamsulosin alone, IPSS were significantly improved at the end of the trial, supporting the evidence for the use of tadalafil in combination with tamsulosin in men with BPH and LUTS
