Adolescents living with HIV in sub-Saharan Africa face challenges in maintaining adherence to antiretroviral therapy (ART). In high-prevalence settings such as Homa Bay County, Kenya, household dynamics and the disclosure of HIV status are critical factors influencing adolescent adherence, yet they remain under-researched. This data descriptor documents a curated dataset from a study investigating the effect of a structured, family-centred HIV disclosure intervention on ART adherence among adolescents aged 10–14 years living with HIV in Homa Bay County. The dataset originates from a longitudinal, mixed-methods study. Quantitative data on adherence (measured via pill counts and self-report) and psychosocial metrics were collected at multiple timepoints. Qualitative data from semi-structured interviews with adolescents and their caregivers explored experiences of disclosure and treatment. The dataset comprises de-identified participant records, survey responses, and anonymised interview transcripts. The curated data indicate an association between the structured disclosure intervention and improved adherence metrics. Analysis shows a notable proportion of participants in the intervention arm achieved adherence levels above 95% at follow-up, compared to a lower proportion in the standard care group. Qualitative data highlight themes of reduced internalised stigma and improved caregiver–adolescent communication post-intervention. This dataset provides a resource for examining the relationship between planned HIV status disclosure and adolescent ART adherence within family contexts. It establishes a foundation for secondary analysis of psychosocial determinants of health in paediatric HIV care. Researchers are encouraged to use this dataset for further analysis on mechanisms linking disclosure to adherence, for investigating long-term outcomes, and to inform the development of family-based support programmes. Ethical reuse requires adherence to the stipulated data access agreements.
