Case: A 33-year-old obese man presented with knee pain and instability after an injury at a trampoline park. He had a patellar tendon rupture, ACL rupture, and MCL rupture. He underwent a 2-stage reconstruction; intraoperative findings required shifting from the initial plan of primary patellar tendon repair to reconstruction using a hemipatella patellar tendon allograft with tibial tubercle bone block. Conclusion: The objective of this case report was to describe a segmental patellar tendon defect initially planned for primary repair but found unamenable intraoperatively due to the subacute injury and inability to mobilize the distal patellar tendon, requiring graft reconstruction.
