This report describes a minimally invasive approach to address midline deviation and gingival discrepancies in a 50-year-old patient seeking rapid aesthetic results. Treatment included prosthetic crown lengthening, selective root modification, and biologic shaping to correct a 3 mm midline shift. Digital smile design and CAD/CAM technology guided the fabrication of zirconia restorations, achieving improved midline alignment, gingival harmony, and smile aesthetics with high patient satisfaction. This technique provides an efficient alternative for complex aesthetic cases.
