Urinary albumin measurement is vital in the screening and monitoring of diabetic kidney disease, but is substantially under-performed. We analysed pathology data from St Vincent's Pathology, Melbourne, between 2022 and 2024 to assess urinary albumin-creatinine ratio (ACR) testing rates in diabetes. Among 19 198 adults, only 23.7% had ≥1 ACR value recorded. Testing increased with risk markers (age, male sex, high hemoglobin A1c, low estimated glomerular filtration rate) but remained low, underscoring significant screening and monitoring gaps.
