Resumen

症例は20代男性．2021年発症のクローン病（Crohn’s disease;以下CDと略記）に対して近医で5-アミノサリチル酸製剤の投与と成分栄養療法で経過観察中であった．経過中に腸閉塞症状を繰り返すようになり，2023年１月よりアダリムマブの投与が開始されたが症状の改善を認めず，手術目的で当院に紹介となった．小腸造影検査と下部消化管内視鏡検査では回腸末端に狭窄を認めた．胸腹部単純CT検査を施行したところ，胸腹部の内臓全体が左右逆に位置している完全内臓逆位と診断した．患者が若年で開腹手術の既往がなかったことも考慮し，患者本人に説明を行ったうえで単孔式腹腔鏡手術の方針とした．骨盤内に回腸の癒着を認めたが，開腹移行はなく単孔式腹腔鏡下回盲部切除術を施行できた．術後経過は良好で明らかな術後合併症は認めず第15病日に退院となった．解剖学的異常を伴うCD症例に対しても単孔式腹腔鏡手術は選択肢の一つである．
