We report a case of meningioma diagnosed in a patient who presented with refractory otitis media. The patient was a 53-year-old woman who presented to our hospital with the complaints of dizziness, earache, and a feeling of obstruction in the left ear. In the supine and suspended head position, rightward horizontal nystagmus was observed, and the left tympanic membrane showed bulging and vasodilatation. Pure tone audiometry (PTA) revealed moderate hearing loss at 45 dB, but auditory brainstem response test showed indistinct V waves at 90 dB. Based on these findings, we suspected retrocochlear deafness. A temporal bone CT scan showed destructive bone lesions in the posterior aspect of the left petrous bone, and a head MRI showed a mass lesion with a uniform contrast effect in the left cerebellar bridge angle, extending into the middle ear cavity. At the time of the operation, the tumor was also found to extend into the internal auditory canal and jugular foramen. Postoperative histopathological examination revealed the diagnosis of meningioma.
