Abstract: The etiology of acute pancreatitis is multifactorial. Hypertriglyceridemia is the third most common cause of acute pancreatitis. Biktarvy, a recently approved human immunodeficiency virus (HIV) treatment, is a single tablet regimen in combination with bictegravir, emtricitabine, and tenofovir alafenamide (BIC/FTC/TAF). A multitude of studies have identified a high risk of weight gain and hyperlipidemia, a trigger for acute pancreatitis, with Biktarvy use. This is the inaugural case of acquired immunodeficiency syndrome (AIDS) with acute pancreatitis as a secondary complication of Biktarvy and alcohol exposure, accompanied by cholelithiasis and diabetes.
