Objectives: To compare the accuracy of mesiodistal and buccolingual angulation of dental implants placed using two different surgical guide designs. Methods: Radiographic templates were fabricated and preoperative computed tomography scans were used to determine proposed implant angulations. Two types of surgical guides were utilized for implant placement. Postoperative CT scans were obtained and angular deviations were measured using Syngo-CT 2010B software. Statistical analysis was performed using Student’s t-test and Z-test. Results: Mean buccolingual angular deviation was 5.90 ± 3.27° for surgical guide-1 and 6.70 ± 2.64° for surgical guide-2. Mean mesiodistal angular deviation was 5.50 ± 2.83° for surgical guide-1 and 8.10 ± 4.45° for surgical guide-2. Intergroup differences were not statistically significant (p > 0.05). However, both guides demonstrated statistically significant deviations from the planned implant angulations. Conclusions: Both surgical guide systems exhibited comparable accuracy in implant placement. Nevertheless, clinically significant deviations from planned angulations were observed, highlighting the need for cautious interpretation of guided implant placement accuracy.
