We report the case of a 31‐year‐old male with COVID‐19 and bilateral central pulmonary embolism. Following interventional thrombus aspiration, a transthoracic echocardiogram revealed the finding of a thrombus‐in‐transit stuck in the patent foramen ovale. Emergency surgical resection was performed without complications. The patient could be discharged without any neurological impairment.
