【摘要】 目的: 探讨2型糖尿病(type 2 diabetes mellitus,T2DM)患者运用基于持续葡萄糖监测(continuous glucose monitoring,CGM)的社区联动管理模式对血糖达标率的影响。 方法: 选取2023年1月—2025年2月于上海市徐汇区徐家汇街道社区卫生服务中心糖尿病专病门诊就诊的100例T2DM患者,采用随机数表法分为对照组和观察组,各50例。对照组患者采用常规干预方案,观察组患者采用基于CGM的社区联动管理方案,比较两组患者血糖水平、自我效能感、膳食指标、服药依从性、负性情绪、生活质量及血糖达标率。 结果: 干预后,相较于对照组,观察组患者空腹血糖、餐后2 h血糖、糖化血红蛋白水平均更低(PPPPPP 结论: 针对临床T2DM患者,采取基于CGM的社区联动管理模式,可使血糖指标保持在稳定状态,提高自我效能感,增强服药依从性,降低负性情绪水平,提高生活质量,提升血糖达标率。
