A 30-year-old P2L2 woman presents to the emergency department on postoperative day-8 (POD-8) after an uncomplicated lower segment cesarean section (LSCS) performed for non-progress of labor.Her antenatal period was notable for gestational diabetes mellitus (GDM) on insulin and a body mass index (BMI) of 32 kg/m.Surgery was uneventful, and she was discharged on day 3.She now complains of persistent high-grade fever for 4 days; dull, continuous pain in the right lower abdomen, radiating to the flank; mild nausea but no vomiting.She has already received broad-spectrum intravenous antibiotics (piperacillin-tazobactam + metronidazole) for suspected postpartum endometritis for the past 2 days at a peripheral center, with no clinical improvement.
