Progressive outer retinal necrosis (PORN) represents the continuum of a fulminant, viral-associated necrotising retinitis in the immunocompromised host. The rapid course of PORN and optic atrophy is most consistent with a herpetic viral retinitis most commonly associated with varicella zoster virus and, less commonly, herpes simplex virus or cytomegalovirus; mumps associated cases are exceedingly rare. We report a rare and challenging case of bilateral PORN following mumps infection in an immunocompetent patient, treated with antiviral agents, steroids, and plasmapheresis, with preservation of central visual acuity and ambulatory visual fields. There have been only two other cases of outer retinal necrosis reported in the literature following fever and parotitis, both of which showed significant vision deterioration, to nearly the absence of perception of light.
