RITMOCORE deals with the technological evolution in the treatment of patients with arrhythmias using or inneed of a pacemaker.RITMOCORE aims to transform the care services for pacemaker patients from a model centred on thepacemaker implanting hospital to a patient centred model and is looking for ICT services and solutions.At this stage of the analysis, the RITMOCORE approach is intended to promote a comprehensive pattern ofservices that could include, in relation to the local context of each procurer: the active involvement of GPs and integration of care pathways through adequate informationsharing; the patient education, leading to an effective activation; the remote monitoring of pacemaker; the mediation of a support centre, to reinforce the contacts with the patient, pre-process the remote data and triage the patient’s needs.
